Wentworth Health Partners is a new formation of primary and specialty care practices that have come together as a partnership in order to better coordinate patient care throughout our system. You will begin to see our name listed as Wentworth Health Partners South Berwick Family Practice!
After Hours Clinic
Wentworth Health Partners is pleased to announce our new After Hours Clinic that offers appointments for acute/sick visits on Saturday mornings. Appointments are available to established patients of any Wentworth Health Partners primary care practice. The After Hours Clinic is located at 10 Members Way, Suite 203 in the Wentworth Professional Building in Dover, NH. The hours are 8:00 A.M. to 12:00 P.M. and appointments can be booked three ways:
1. Book a Saturday morning acute/sick appointment through your regular provider's office beginning Friday at 12:00 P.M.
2. Call your primary care provider's office Saturday morning and use the prompt for the nurse triage or provider on-call service to request an appointment time.
3. The After Hours Clinic Phone Line is available on Saturday from 8:00 A.M. to 12:00 P.M. at 603-609-6930.

We now offer the NextMD Patient Portal! Sign up at the office today or download the Patient Portal Consent Form and return it to our office. The Portal is a secure website that allows access to the following:
ePrescribing Available
Wentworth Health Partners South Berwick Family Practice is pleased to announce the availability of ePrescribing. E-Prescribing is a computer-based transmission and filling of a prescription between your provider and pharmacy. We encourage you to contact your pharmacy directly for refills to facilitate a faster turnaround time for refill requests.

We are now providing Medicare Annual Wellness Exams. Refer to the attached document on Frequently Asked Questions and the attached Annual Wellness Exam Questionnaire. Please contact the practice if you have any additional questions regarding this exam and eligibility or to schedule your appointment.
